Registration Instructions

To ensure proper enrollment, please complete the appropriate forms based on the student’s age group:

• Teen (16 &17) Registration: Fill out Form 1 and Form 2

• Adult (18 & older) Registration: Fill out Form 1 and Form 3

All required forms must be submitted before any payment is processed. Once your forms are received and payment is confirmed, we will contact you to begin scheduling your appointments.
